The Claim Process
KEMI policyholders are responsible for reporting injuries or illnesses in a timely manner; however, the injured worker may submit his or her own First Report of Injury using the IA-1 First Report of Injury form.
Once a work-related injury is reported to KEMI, our Claims Team will move into action by reviewing the information and ensuring the injured worker receives the care they deserve.
Once we’ve reviewed the First Report of Injury, we will gather additional information and determine appropriate benefits if the injured worker is expected to miss work or needs ongoing treatment from a provider in our network. If the injured worker is able to return to work with restrictions, we will work with the policyholder to make those accommodations if possible.
As part of our commitment to you, we want to help you return to work as quickly and safely as possible.

Frequently Asked Questions
Workers’ compensation insurance (workers’ comp) is a form of accident insurance paid by employers which covers an employee’s medical expenses and lost wages if they are injured on the job or acquire a work-related illness.
Work-related injuries and illnesses should be reported to KEMI immediately. Kentucky law states that policyholders must report injuries or illnesses to us within three (3) working days of receiving notification of the incident or alleged incident.
The policyholder is responsible for reporting the injury to KEMI in a timely manner; however, the injured worker may submit his or her own First Report of Injury.
An injured worker may report a claim by submitting the First Report of Injury or Illness, or IA-1 form, and faxing it to 859-425-7822 or mailing to the address below:
