HOW CAN I REPORT AN INCIDENT TO KEMI?

Registered policyholders can log in and submit a First Report of Incident at KEMI.com.

Not registered? Policyholders can go here to register for access to our online services. Otherwise, incidents should be reported on an First Report of Incident Form and sent as an attachment to clmsfroi@kemi.com.

The First Report of Incident Form may also be submitted by phone during business hours at 800-640-5364, by fax at 859-425-7822 or by mail to KEMI at P.O. Box 12500, Lexington, KY 40583-2500.

HOW CAN AN CLAIMANT FILL A PRESCRIPTION?

Claimants should take prescriptions to a pharmacy and tell the pharmacist to bill the prescription through KEMI’s third-party pharmacy network provider, Mitchell.

Employers must notify KEMI within three (3) working days of notice of an alleged incident or illness.

KEMI’s claims staff will contact the policyholder and/or claimant if additional information is needed. The policyholder and/or claimant may contact KEMI’s claims staff with any questions or concerns by calling 1-800-640-5364.

Medical and indemnity (lost wage) benefits may be provided to employees who have a work related incident. The benefits provided for work related incidents vary depending on the specific facts and circumstances of each claim. Please contact KEMI’s claims staff at 1-800-640-5364 for questions regarding a specific claim.

There are three types of indemnity benefits: temporary total disability (TTD), permanent partial disability (PPD) and permanent total disability (PTD). Each type of indemnity benefit has a different calculation defined by statute and uses specific information related to the claim. Please contact KEMI’s claims staff at 1-800-640-5364 for questions regarding a specific claim.

Under Kentucky law (KRS 342.038), all claims must be reported to us within 
three (3) working days of your knowledge of any work-related injury or illness.
